Proposal to merge Fountain Medical Centre and Dr N Saddiq’s Surgery.

What is this survey about?

Fountain Medical Centre and Dr Saddiq’s Surgery have submitted an application to NHS West Yorkshire Integrated Care Board (ICB) to merge both practices onto one site at Fountain Medical Centre.  This will ensure the future ability to deliver high quality primary care to our patients.

NHS West Yorkshire Integrated Care Board (ICB) are responsible for planning and funding most of your local healthcare services and have agreed for this involvement to take place.

The merger will offer improved access and choice for patients.  Working together from Fountain Medical Centre will mean that both practices are better placed to deliver high quality primary care services in the future.

This survey gives you the opportunity to get involved in this change.  Your responses will help us to understand what is most important to you about the GP practice service you receive and your views and thoughts on this merger. 

We are keen to hear your feedback and comments about this proposal.  The survey will be open until midnight on 18 November 2022.

There are a few ways that you can share your thoughts about the merger:

  • By attending a face-to-face public meeting and a virtual MS teams meeting where you can hear more about the merger and submit questions in advance.  We will take note of your questions and comments at this meeting, and they will be added to the feedback other people give through the survey to contribute to our patient involvement report.

The face-to-face meeting will be held at Morley Cricket Club, Scatcherd Lane, Morley, LS27 0JJ on Monday 31 October 11:30am – 1:00pm.  The virtual meeting will be held on Wednesday 9 November 5:00pm – 6:00pm.

We will post updates about the involvement exercises on our website.

The final patient involvement report will be available on our website and will be shared with NHS West Yorkshire ICB to support them to understand if there are any common themes in relation to the provision of services that may need to be considered in future planning.
